This session will explore the key tenets of successful primary care with a panel representing several delivery options.   Primary care for working Americans is at a crossroads. The U.S. faces a projected shortage of over 87,000 primary care physicians by 2037.  At the same time, over half of PCPs report burnout due to heavy workloads and low compensation.  Weakened primary care leads to more fragmented and reactive care, increasing downstream costs for employers, specifically costs associated with chronic conditions and high-cost claimants.  Employers can address these challenges by investing in innovative primary care models that improve access, leverage technology, and focus on prevention and outcomes, which are proven to lower costs and improve care quality.

Advanced Primary Care should occupy the front line in the management of employee health and wellbeing. By reducing barriers to care, screening for issues, preventing illness and catching potential problems before they become acute, Advanced Primary Care reduces the overall cost of care.. The value of Advanced Primary Care is tightly linked to improving population health, reducing total cost of care and increasing employee engagement, however. If employers don’t know how to set up an effective program, employers and employees lose out on the maximum value.

Kelsey-Seybold Clinic & Goose Creek ISD – Primary and Specialty Care Integration

Large employer Goose Creek ISD has implemented an “integrated primary and specialty care” strategy to improve plan member experience and address escalating costs.  To implement the strategy, Goose Creek ISD added a new health plan offering, partnering with Blue Cross Blue Shield and Houston-based ACO Kelsey-Seybold Clinic.   A key feature is “Open Access,” eliminating referrals and empowering members to schedule 24/7 with 1,000+ Primary Care physicians and Specialists who coordinate patient care.   Employee communication has been key to implementation.  Dr. Ellen Akers, Director of Employee Health and Wellness at Goose Creek ISD, will share the District’s implementation plan and experience. Dr. Azam Kundi, Kelsey-Seybold Clinic CEO, will share how clinical integration enables providers to direct care to the appropriate setting, eliminating redundancy and improving value.

The addition of an employer sponsored advanced primary care model has demonstrated measurable success across organizations of varying sizes, including Humble ISD (4,800 employees) and the City of Manvel (100 employees). By customizing implementation strategies, both organizations have achieved strong program utilization—72% for the City of Manvel and 60% for Humble ISD—with outstanding patient satisfaction reflected in Net Promoter Scores (NPS) of 90 or above with Next Level Prime.

Next Level Prime offers members access to care with scheduled appointments both virtually and in person seven days a week from 9am. to 9pm plus 24/7 telemedicine, leading to increased convenience and reduced absenteeism with 25% of visits occurring outside of typical physician office hours.

Care navigation further enhances value by ensuring referrals remain in-network for both clients, minimizing unnecessary expenses. For Humble ISD, care navigation is taken a step further by ensuring that 100% of referrals are sent through a dedicated program for additional patient and employer savings. Overall, both employers have achieved 50–60% cost savings on services provided under the Next Level Prime program compared to traditional self-funded plan expenses.
