The Houston Region Hospital Report Card provides a dashboard composite view hospital’s current Leapfrog Safety Grade, and its commitment to safety and quality improvement as evidenced by its full completion of the annual Leapfrog hospital Survey, and it’s achievement in 12 key metrics included in the annual survey.

Since 2000, Leapfrog has served as America’s voice for patient safety, quality, and transparency. The Leapfrog Hospital Safety Grade provides a consumer-friendly letter grade system evaluating nearly 3,000 hospitals on how well they keep patients safe from harm. The Leapfrog Hospital Survey is a voluntary survey on safety, quality, and resource use completed by over 2,000 hospitals annually. The Leapfrog Ambulatory Surgery Center Survey is a new voluntary survey for ASCs to report on the quality and safety of the care they provide.
Texas & Houston Region Leapfrog Safety Grades
The Leapfrog Hospital Safety Grade uses more than 30 national performance measures from the Centers for Medicare & Medicaid Services (CMS), the Leapfrog Hospital Survey and information from other supplemental data sources. Taken together, those performance measures produce a single letter grade representing a hospital’s overall performance in keeping patients safe from preventable harm and medical errors. The Leapfrog Hospital Safety Grade methodology has been peer reviewed and published in the Journal of Patient Safety.
